Make your first step toward changing your life. By submitting this PAR-Q (Physical Activity Readiness Questionnaire), this information will allow me to develop a customized plan specific to YOUR lifestyle and goals.  When you are completing the PAR-Q, please be completely honest and as detailed as possible so that your plan can be tailored specifically to your goals. Tell me about where you are currently, where you want to be, and what your limits are. Do not tell me what you think I want to hear. All information is kept confidential unless you decide to volunteer your testimony for later use by NLT. If you have any questions, please contact me.

 

Record at least a three day food log. I recommend using www.myfitnesspal.com or downloading the myfitnesspal mobile app. 

IMPORTANT! Please don't change anything that you're eating while initially logging your food intake. I need to know exactly what you're eating now to create your proper macro plan.

Before starting any program, you will need to take a before photo (front, side, & back) so I can track your progress and make as needed adjustments to your plan.
